骨髓间充质干细胞移植治疗骨不连研究进展

摘要:

骨不连的治疗都以自体骨移植等为主,但是由于自体骨取骨对患者损伤较大,可产生供区畸形、顽固性疼痛等并发征,使其临床价值受到一定限制。骨髓间充质干细胞是一类具有自我更新和分化潜能的细胞,在一定的诱导条件下,骨髓间充质干细胞可定向分化为成骨细胞,在骨折的愈合过程中骨髓间充质干细胞起着重要的作用,为提高骨髓间充质干细胞的成骨能力,不同方式联合移植是目前研究的重点。随着组织工程技术的发展,骨髓间充质干细胞复合支架材料在骨不连治疗中取得了良好的效果。可用作组织工程支架材料如括胶原,骨形态发生蛋白,丝素蛋白,羟基磷灰石、磷酸三钙、生物活性玻璃陶瓷,磷酸钙水泥,高分子有机物等。如何选择最佳的支架材料、移植时机、细胞浓度等问题,尚期待更多的研究评估。

Abstract:

Nonunion are treated with autogenous bone graft, etc, but due to autogenous bone graft in patients with bone greater damage can be generated for malformation, complicated by intractable pain and other symptoms, so that the clinical value is limited. Bone marrow mesenchymal stem cells are a class of self-renewal and differentiation potential of the cells, under certain induction conditions, bone marrow mesenchymal stem cells can differentiate into osteoblasts in the healing process of fracture of bone marrow mesenchymal stem cells play an important role for improving bone marrow mesenchymal stem cells into bone capability, in different ways transplantation is the focus of current research. With the development of tissue engineering techniques, bone marrow mesenchymal stem cells in stem cell scaffold material achieved good results in the treatment of nonunion. It can be used as tissue engineering scaffolds include collagen, bone morphogenetic protein, fibroin, hydroxyapatite, tricalcium phosphate, bioactive glass ceramics, calcium phosphate cement, polymer organic matter. How to choose the best scaffolds, transplant timing, cell concentration and other issues, we are still looking forward to more research assessment.
