骨形态发生蛋白:由多种细胞分泌产生的多功能蛋白,可诱导动物或人间充质细胞分化为骨、软骨、韧带、肌腱和神经组织。因具有诱导骨外组织发生软骨内成骨的能力,故被称为骨形态发生蛋白。
软骨再生:通过促进机体自身软骨细胞进行局部增殖,修复磨损或者缺损的软骨,使关节面变平滑,恢复关节软骨原有结构,起到关节保护作用。
背景:关节软骨退变是导致骨关节炎最主要的原因。骨形态发生蛋白在软骨再生和修复中起着重要作用。
目的:综述骨形态发生蛋白在关节软骨再生过程中的作用和应用研究进展。
方法:应用计算机检索 PubMed 数据库及 Elsevier 数据库,检索词为“bone morphogenetic proteins,BMPs,arthritis,osteoarthritis,OA,cartilage,chondrocyte”,共检索到相关文献272篇,最终选取符合标准的文献96篇,对其进行分析、归纳和总结,还有27篇文献为概念类相关文献,最终纳入文献123篇。
结果与结论:骨形态发生蛋白参与了包含细胞增殖、分化、迁移、凋亡等诸多生物学过程,在骨和软骨形成过程中发挥了重要作用。骨形态发生蛋白通过与不同的受体结合,参与了多种信号通路级联反应,保护关节软骨免受炎症、创伤等导致的软骨破坏。骨形态发生蛋白单独或与其他细胞因子联合使用能够修复软骨缺损和改善退行性病变,促进关节软骨细胞的分化和再生。但是目前将骨形态发生蛋白广泛应用于软骨再生还有一些实际问题亟待解决,例如缺乏有效的生物支架材料,使用生物制剂的最优剂量和作用时间点及其毒副作用等。今后的研究将围绕如何解决以上问题开展,骨形态发生蛋白的广泛应用将为软骨损伤和以骨关节炎为代表的软骨退行性病变的靶向治疗开启新的纪元。

文题释义:#br# 骨形态发生蛋白:由多种细胞分泌产生的多功能蛋白,可诱导动物或人间充质细胞分化为骨、软骨、韧带、肌腱和神经组织。因具有诱导骨外组织发生软骨内成骨的能力,故被称为骨形态发生蛋白。#br# 软骨再生:通过促进机体自身软骨细胞进行局部增殖,修复磨损或者缺损的软骨,使关节面变平滑,恢复关节软骨原有结构,起到关节保护作用。
目前有大量研究应用骨形态发生蛋白治疗软骨损伤,并取得了较好效果,但离广泛应用于临床仍有距离。骨形态发生蛋白转运效能的问题亟需解决。如何开发新的生物支架将成为未来研究的关键点。与各种干细胞联合使用,特别是关节腔局部注射,具有较高的临床应用前景。将骨形态发生蛋白与其他生长因子整合进入宿主的原始细胞中,利用宿主自身的干细胞,降低了免疫风险并促进了干细胞分化的效能,在骨形态发生蛋白的刺激下加快了软骨修复进程是未来研究的另一个方向。此外,骨形态发生蛋白结合微骨折技术也被证实能促进骨髓间充质干细胞向软骨损伤部位迁徙,从而加速了软骨修复。
