单侧双通道内镜技术治疗腰椎疾病:全球研究现状及变化趋势

单侧双通道内镜技术:代表了微创脊柱外科手术的一个领域,近年来取得了显著的进步。随着微创手术模式的发展和脊柱内窥镜器械的不断完善,该技术在临床领域获得了越来越多的关注。
文献计量学:将统计方法与数据可视化技术相结合,被用于评价单侧双通道内镜技术应用于腰椎疾病领域的科研生产力和知识结构,为科研方向提供数据支持。

摘要
背景:单侧双通道内镜技术作为一种新的微创脊柱技术,已成功应用于多种腰椎疾病的治疗。
目的:从文献计量学的角度,直观地分析国际上单侧双通道内镜技术治疗腰椎疾病相关研究的热点和发展趋势,以期为今后的治疗策略提供新的见解。
方法:使用Web of Science核心数据库检索和下载2015-01-01/2024-12-31关于单侧双通道内镜技术治疗腰椎疾病相关研究的出版物,使用VOSviewer、CiteSpace以及Microsoft Excel进行文献计量学分析,分析出版物数量、国家/地区、机构、作者、期刊、参考文献和关键词,确定显著特征,为今后单侧双通道内镜技术治疗腰椎疾病提供新的见解和科研思路。
结果与结论:①共纳入265篇文献,年度出版物数量稳步增加,这些文献主要来自以中国为首的56个国家/地区的706家机构,韩国的翰林大学的出版物最多,Dong Hwa Heo是发文量最多的作者,《WORLD NEUROSURGERY》是出版物最多的期刊;②高被引文献分析结果展示了单侧双通道内镜技术的优越性,其被认为是腰椎退行性疾病的一种替代治疗方法;③关键词分析显示,减压是出现频率最高的关键词;关键词主要被分为8个集群:内镜脊柱手术、双侧减压、背肌损伤、微创手术、椎间孔狭窄、单侧双通道内镜技术、研究趋势、内镜手术;④单侧双通道内镜技术治疗腰椎疾病的相关研究近年来呈现上升趋势,主要用于减压术、椎间盘摘除术、椎板摘除术,主要针对的疾病是腰椎椎管狭窄症和腰椎间盘突出症;该领域的研究仍处于起步阶段,学者在不断探索单侧双通道内镜技术的适应手术及临床效果的同时,各种并发症也备受关注;⑤近年来该领域的研究热点集中在单侧双通道内镜技术治疗邻近节段退变性疾病,单侧双通道内镜技术在全球范围内普及率仍然较低,国情和高层次技术人员的培养可能是其限制原因;未来国内应强化国际学术合作与技术引进、放缓技术扩张脚步、提高研究质量、规范操作、制定专家共识;大量的前瞻性随机对照试验是必要的。

Abstract: BACKGROUND: As a new minimally invasive spine technique, unilateral biportal endoscopic technology has been successfully applied to the treatment of various lumbar diseases.
OBJECTIVE: From the perspective of bibliometrics, this paper was to intuitively analyze the hotspots and research trends of unilateral biportal endoscopic technology in the treatment of lumbar diseases in the world, in order to provide new insights for future research. 
METHODS: Web of Science Core Collection database was used to retrieve and download the publications on the application of unilateral biportal endoscopic technology in the treatment of lumbar diseases from January 1, 2015 to December 31, 2024. VOSviewer, CiteSpace, and Microsoft Excel were used to conduct this bibliometric analysis. The publication volume, countries/regions, institutions, authors, journals, references, and keywords were analyzed to identify significant features, providing new insights and scientific research ideas for the future use of unilateral biportal endoscopic technology in the treatment of lumbar degenerative diseases.  

RESULTS AND CONCLUSION: (1) A total of 265 articles were included, and the number of annual publications increased steadily. These documents mainly come from 706 institutions in 56 countries/regions headed by China. Hallym University in Korea has the most publications. Dong Hwa Heo is the author who has published the most articles. WORLD NEUROSURGERY is the most published periodical. (2) The analysis results of highly cited and strong sudden references show the superiority of unilateral biportal endoscopic technology, which is considered as an alternative treatment for lumbar degenerative diseases. (3) Keyword analysis showed that decompression was the most frequent keyword. Key words are mainly divided into eight clusters: endoscopic spinal surgery, bilateral decompression, back muscle injury, minimally invasive surgery, foraminal stenosis, unilateral biportal endoscopic technology, research trends, and endoscopic surgical procedure. (4) The research on the treatment of lumbar diseases by unilateral biportal endoscopic technology has shown an upward trend in recent years. Unilateral biportal endoscopic technology is mainly used in decompression, discectomy and laminectomy, and its main diseases are lumbar spinal stenosis and lumbar disc herniation. The research in this field is still in its infancy, and scholars are constantly exploring the adaptation of unilateral biportal endoscopic technology to surgery and clinical effects, while its complications have attracted much attention. (5) In recent years, the research focus in this field has focused on unilateral biportal endoscopic technology in the treatment of adjacent segmental degenerative diseases. The popularization rate of unilateral biportal endoscopic technology in the world is still low, and the national conditions and the training of high-level technicians may be the limiting reasons. In the future, China should strengthen international academic cooperation and technology introduction, slow down the pace of technology expansion, improve the quality of research, standardize operations and formulate expert consensus. A large number of prospective randomized controlled trials are needed.
