摘要：

背景：椎体环截是近年来提出较多的脊柱矫形概念，因其操作技巧要求高，难度大，并发症多，既往相关文献报道角度各不相同，很多研究者注重并发症分析，究其原因与操作步骤及方式有关。
目的：分析单纯后路椎体环截及螺钉置入治疗重度脊柱畸形的疗效和并发症。
方法：回顾性分析48例单纯后路椎体环截及螺钉置入治疗重度脊柱畸形患者的临床资料，平均切除椎体1.6个，随访15-64个月，分析所有患者治疗前、治疗后及最后一次随访时Cobb角(冠状面及矢状面)，并对手术相关并发症进行分析。
结果与结论：纳入的脊柱畸形患者共分5类，其中脊柱侧后凸畸形11例，重度侧凸畸形20例，先天性脊柱畸形4例，球状后凸畸形3例，角状后凸畸形10例。所有患者治疗前平均冠状面畸形角度由84°矫正到35°，总的矫正率达到54%。治疗前平均矢状面畸形角度由90°矫正到42°，矢状面 Cobb 平均减小了48°。手术时间平均545 min(204-1 355 min)，术中平均估计失血量 1 610 mL(50-8 244 mL)，平均失血量达65%。31例治疗后出现并发症，其中13例术中神经功能异常(通过电生理监测及唤醒实验发现)，及时处理后患者未出现永久性神经功能障碍。提示单纯后路椎体环截及螺钉置入治疗重度脊柱畸形效果良好，但并发症高，操作难度较大。

Abstract:

BACKGROUND: Vertebral column resection is the frequently mentioned spinal orthopaedic concept. Due to the high requirement of the operation skill, difficulty and more complications, the previous studies have reported from different aspects, and many researchers have focused on the analysis of complications, that may be related with the procedure and manner.
OBJECTIVE: To analyze the efficacy and complications of posterior vertebral column resection combined with titanium screw rod fixation for the treatment of severe spinal deformity.
METHODS: We retrospectively analyzed 48 patients with severe spinal deformity who treated with posterior vertebral column resection and titanium screw rod fixation, with an average removal of 1.6 vertebral. The patients were followed-up for 15-64 months. The Cobb angle (coronal plane and sagittal plane) of the patients before treatment, after treatment and in the final follow-up was analyzed, and the relative complications of the surgery were analyzed.
RESULTS AND CONCLUSION: The patients with spinal deformity were divided into five categories, included kyphoscoliosis (n=11), severe scoliosis (n=20), congenital spinal deformity (n=4), spherical kyphosis (n=3), and angular kyphosis (n=10). The average coronal plane deformity angle of the patients was corrected from 84° preoperation to 35° postoperation, with the total correction rate of 54%. The average sagittal plane deformity angle was corrected from 90° preoperation to 42° postoperation, and the sagittal plane Cobb angle was decreased for 48°. The mean operation time was 545 minutes (204–1 355 minutes), the intraoperative blood loss was 1 610 mL (50–8 244 mL), and the average blood loss was 65%. After treatment, 31 cases had complications, including 13 cases of intraoperative neurological dysfunction (observed through electrophysiological monitoring and wake), permanent neurological dysfunction did not occur after timely treatment. The posterior vertebral column resection and titanium screw rod fixation can obtain better effect in the treatment of severe spinal deformity, but the procedure has high complications and is difficult for operation.

2.1 参与者数量分析按意向性处理分析，纳入48例患者，全部进入结果分析，无脱落。随访时间15-64个月。
2.2 患者手术相关资料病例中的脊柱畸形包括5类：重度侧凸、球状后凸、角状后凸，侧后凸和先天性畸形，术中平均切除椎体1.6个(1-5个)，平均手术时间545 min，平均估计失血量1 610 mL，失血比例达65%，见表1。

2.3 影像学分析结果所有患者治疗前平均冠状面畸形角度由84°矫正到35°，总的矫正率达到54%。治疗前平均矢状面畸形角度由90°矫正到42°，矢状面 Cobb 平均减小了48°。
48例患者中脊柱侧后凸畸形11例，重度侧凸畸形20例，先天性脊柱畸形4例，球状后凸畸形3例，角状后凸畸形10例，收集所有患者治疗前、治疗后平均直立、屈曲位下Cobb角度数值(矫正率及平均后凸角度减小度数)。
对于重度侧弯的患者冠状面侧弯Cobb角在侧曲位片上由 104°减小为78°，治疗后首次拍片冠状面平均Cobb角为32°，最后一次随访为33°，平均矫正率为67%。
对于球面畸形的患者治疗前平均最大矢状面后凸Cobb角度101°，治疗前过伸位为79°，治疗后首次平均矢状面Cobb角46°，最后一次随访为47°，平均后凸角减小54°。
对于角状后凸畸形患者，治疗前平均最大矢状面Cobb角88°，屈伸位片几乎没改善，治疗后首次拍片矫正到39°，最后一次随访为38°，后凸角平均减小50°。
对于侧后凸患者，治疗前冠状面平均Cobb角为91°，侧曲位片减小为65°，治疗后首次平均冠状面Cobb角43°，最近一次随访平均Cobb角为44°，冠状位平均矫形率达51%；治疗前矢状面平均Cobb角104°，治疗后平均首次47°，最后一次随访无变化，后凸角度平均减小57°。
对于先天性脊柱畸形患者，治疗前冠状面平均Cobb角度47°，治疗后首次拍片显示治疗后平均矫正到22°，最后一次无变化，矫形率为46%；治疗前平均矢状面Cobb角56°，治疗后首次为30°，最后一次随访为32°，后凸角减小了24°。
2.4 并发症见表2。

共有31例患者出现并发症，最常见的术中并发症是术中神经电生理监测异常，结合唤醒实验提示急性脊髓神经损伤，其次最常见的术中并发症为术中估计失血量多于2 L，最常见治疗后早期并发症为呼吸系统相关并发症，包括肺部感染、重度肺不张、胸腔积液和切口感染。治疗过程中及治疗后无患者死亡。
2.5 SRS-22评分结果所有患者治疗前及治疗后1年内应用SRS-30评分量表评价疗效，治疗前平均为3.6 (2.4-4.5)分，术后平均4.2(2.5-4.7)分，差异有非常显著性意义(P < 0.001)。
疼痛评分改善由4.0分到4.3分(P=0.136)，外观自我评价从2.8分改善到4.2分(P < 0.001)，功能改善从3.8分到4.1分(P=0.023)，精神心理状态从3.8分改善到4.1分(P=0.035)，满意度改善从3.0分改善4.3分(P < 0.001)，所有患者治疗后评分均较治疗前明显改善。
2.6 典型病例见图1，2。

图1，2所示为5岁女性患儿，脊柱重度侧弯，行单纯后路T11椎体环截T2-L4椎弓根螺钉内固定融合，术中神经电生理监测及唤醒试验。治疗后冠状面矫正到39°，矢状面矫正到+30°，矫正效果良好；治疗后冠状面及矢状面均显示了良好的躯干平衡。

引言

材料方法

设计：回顾性病例分析。

时间及地点：于2004至2012年在新疆医科大学附属中医医院脊柱科一组完成。

对象：收集新疆医科大学附属中医医院脊柱科一组2004至2012年收治的重度脊柱畸形行单纯后路椎体环截及螺钉置入内固定治疗患者48例，男30例，女18例，平均年龄13.7岁；平均切除椎体1.6个(1-5个)；随访15-64个月。

回顾性分析所有患者治疗前、治疗后及最后一次随访时影像学测量的Cobb角(冠状面及矢状面)，参阅病例资料提供的治疗相关并发症。

纳入标准：①脊柱侧后凸畸形超过80°的僵硬型脊柱畸形患者。②先前未做过脊柱手术者。③对治疗方案知情同意，且得到医院伦理委员会批准者。

排除标准：①无法耐受手术者。②合并其他脊柱疾患者。

后路钛合金钉棒内固定系统：购自美国枢法模器械公司，螺钉直径3.5-5.5 mm，长度3.0-5.0 mm，钛棒直径为65 mm，长度依据固定节段长度确定，成分为钛-钒-铝合金，材料弹性模量11.7*10 kA，伸长率为12%，抗疲劳程度55 160 kP。材料已经过测试，无细胞毒性、血液及组织相容性问题。

方法：

治疗前准备：气管插管全身麻醉后，将患者俯卧位于弓形架上，C形臂机透视确定责任节段。

治疗过程：沿脊椎棘突做后正中纵行切口，依次切开皮肤、皮下及深筋膜，剥离两侧椎旁肌。骨膜下剥离暴露手术范围内椎体棘突、椎板、横突、小关节等后方结构，胸椎需暴露至肋横突关节外侧。在环截椎体上下临近节段置入椎弓根螺钉。椎弓根钉进针点通常为(肋)横突中点的水平线与上关节突外缘切线的交点处。螺钉植入后需用C形臂机透视查看螺钉植入位置是否满意。于凹侧置放预弯至生理曲度的预置棒固定，以防止截骨操作时剪切力损伤脊髓。快速静脉滴注奥美拉唑40 mg及甲基强的松龙1 000 mg，以减轻后续暴露、减压等操作所可能导致的神经水肿及缺血再灌注损伤，并预防应激性胃溃疡。切除需环截椎体后方结构，包括韧带、关节突、椎板等，显露硬脊膜及对应神经根。对胸椎畸形患者，还需切除3-5 cm相应节段的包括肋骨头在内的小段肋骨，以增加操作空间，防止影响矫形操作。沿环截椎体外侧行骨膜下剥离，达到椎体前方。将硬膜囊适度牵拉向凹侧，保护神经根，小心仔细刮除椎体及临近间盘组织和软骨终板，适当松开凹侧预置棒的固定螺帽，于凸侧置放预弯至生理曲度的矫形棒，适当锁紧螺帽，缓慢旋棒，将钛棒的预弯角度转换为脊柱的生理曲度，同时矫正椎体旋转。于凸侧缓慢加压，逐渐闭合残留间隙。加压闭合时，若感觉张力过大，为防止椎弓根螺钉切割椎体，可将固定融合节段向一端或两端延长一个椎体。适度撑开凹侧，锁紧螺帽，探查椎管内硬膜囊有无受压及皱缩，探查相应节段神经根有无牵拉等情况，并给予相应的处理。观察矫形效果，满意后安放横联，临近节段的椎板及小关节制造植骨床，植入自体骨及同种异体骨条，彻底止血后放置引流管1根，然后逐层关闭手术切口。

治疗过程中的注意事项：术中出血主要发生在截骨过程中，通常为硬膜外静脉丛出血及椎体外侧的节段血管出血，可通过双极电凝、止血纱布及明胶海绵填塞配合纱布压迫止血。手术全程行体感诱发电位，神经监护系统实时监测，降低神经系统受损的风险。

主要观察指标：观察指标包括治疗前、治疗后患者Cobb角，手术操作时间、术中失血量、随访时间。所有患者治疗前及治疗后1年内以SRS-30评分量表评价疗效。

SRS-22评分量表包括30个项目，各个项目均是1-5的5点评估，5分代表极好，1分代表极差。问卷涉及以下5个因素：功能活动(5，9，12，15，18)、疼痛(1，2，8，11，17)、自我形象(4，6，10，14，19)、心理健康(3，7，13，16，20)及对治疗的满意程度(21，22)。

讨论

治疗重度脊柱畸形的传统术式是分期前路松解后路融合，针对重度脊柱侧凸、后凸、前凸及混合型脊柱畸形的患者这仍是常用治疗方式，尽管椎体环截手术方式早有报道，但近年来Suk和Lenke等将其用于重度脊柱畸形的患者^[14-22]，尤其是通过单纯后路一期完成，尽管相关报道不少，但对于这种术式的适应证、结果、并发症尚无系统描述报道。而且文献中对于脊柱环截的定义尚有争议。既往报道的截骨方式包括有限后柱截骨、椎弓根截骨、椎体切除及后柱完全截骨即切除椎体后方韧带和关节突，就是所谓的后柱截骨^[23-28]。而三柱截骨包括椎弓根切除和椎体切除^[29-34]，需切除后方韧带，关节突，椎弓根和不同程度椎体及相邻的椎间盘组织。在此，需要准确描述椎体环截的定义，椎体环截的操作过程及其风险不同于先天性脊柱半椎体畸形的切除，这一点毋庸置疑。椎体环截过程中，责任节段最终缺失，脊髓环形暴露，必须内固定。所以，作者认为脊柱椎体环截应定义为：脊柱椎体三柱切除后节段缺失，在操作过程中为避免局部失稳或脊髓剪切平移，需临时内固定责任节段。文章中所选择病例内固定均为椎弓根螺钉钛棒固定系统^[35-42]，其中椎弓根截骨和椎体切除对于先天性脊柱畸形的患者而言较为困难，因椎弓根及相邻椎间盘可能发育不全而缺失。也可能1个或多个椎体节段先天性融合，椎体环截是将脊柱一分为二，然后闭合间隙用于矫形。但椎弓根切除截骨则不同，截骨后仍可维持脊柱稳定性。

所以作者定义的椎体环截是脊柱椎体三柱切除后节段缺失，在操作过程中为避免局部失稳或脊髓剪切平移，需临时内固定责任节段。精确地描述有助于提高术中对此技术的认识，也有助于在顶锥环截过程中头尾端椎体结构的辨认。作者收集数据表明此治疗方案适用于各个年龄段的早期患者，因手术时间短，创伤小，所有患者均通过单一后路行椎体环截椎弓根螺钉内固定矫形治疗。平均手术时间接近9 h，平均失血量超过1 600 mL，但此种方案矫形效果良好。需注意的是对于重度脊柱畸形的患者冠状面的Cobb角度并不能完全用于评价矫正疗效，尤其曾行手术治疗的患者更是如此。因精确测量Cobb角度困难，作者认为X射线片上表现的Cobb角度通常并不总是能精确描述矫形效果，所以治疗前和治疗后还补充用SRS评分量表进行评价。

并发症与原发疾病的严重程度有关，尤其在椎体环截中常见，所有患者没有因并发症死亡的病例，最常见的术中并发症是神经电生理监测和唤醒实验证实脊髓及神经损伤，积极处理后，避免了神经不可逆损伤。许多患者因脊柱畸形导致胸廓畸形，治疗后容易出现肺部并发症，为防止感染可预防性使用抗生素，偶尔因胸膜术中破裂，需胸腔闭式引流，因手术时间较长，治疗后感染率增高，尤其是畸形复杂的患者，操作时间长，更容易发生感染。文章的缺陷是回顾性研究，手术操作者系同一人，而无法提供更多的经验，或许前瞻性的研究能解释更多的问题。其次，随访时间不够长，无法明确更长时间后的矫形效果。

综上所述，经单纯后路椎体环截椎弓根螺钉内固定融合矫正脊柱畸形虽操作复杂，并发症发生率较高，但矫形效果良好，冠状面及矢状面都获得了良好的平衡，尤其对于重度僵硬性脊柱畸形来说是不错的选择。分析相关并发症有助于在进行环截时更仔细。
