Abstract: BACKGROUND: Imaging examinations can be applied throughout the entire diagnostic and treatment process for osteonecrosis of the femoral head. In recent years, emerging technologies such as imageomics, deep learning, and multimodal quantitative imaging have developed rapidly, driving research on imaging examinations for osteonecrosis of the femoral head from traditional morphological descriptions toward intelligent and precise approaches; however, a systematic review of the knowledge framework and analysis of evolving trends remain insufficient.
OBJECTIVE: To use bibliometric and visualization analysis methods to describe the current state of domestic and international research, the knowledge structure, collaborative networks, and evolving trends in the field of imaging examinations for osteonecrosis of the femoral head from 2017 to 2026.
METHODS: We searched the WanFang Database and the Web of Science Core Collection from January 1, 2017 to July 6, 2026, for studies on imaging examinations of avascular necrosis of the femoral head. Ultimately, 1 208 Chinese-language articles and 1 209 English-language articles were included. VOSviewer 1.6.20 software was used to perform co-occurrence, clustering, and overlay visualization analyses of keywords. Research hotspots in this field were comprehensively assessed through co-occurrence networks of journals, institutions, and keywords, as well as bibliometric analysis of highly cited articles.
RESULTS AND CONCLUSION: (1) The number of publications from China has shown a decline trend, while the number of international publications has been on the rise year by year. Guangzhou University of Chinese Medicine is the institution with the highest number of publications in this field, both domestically and internationally. This suggests that research in this field in China may have gradually shifted toward publication in English-language SCI journals in recent years, indicating a structural transition in publication trends. (2) Trends in Chinese keywords have shifted from traditional terms such as “CT, magnetic resonance imaging, and diagnosis” (average years: 2019–2020) to “multi-slice spiral CT, early diagnosis, osteonecrosis, and bone marrow mesenchymal stem cells” (average years: 2021–2022). “Diagnostic efficacy” is the current core theme, while “femoral neck fracture” has recently emerged as a new hot topic. Highly cited Chinese publications focus on comparisons of CT and MRI performance, the use of micro-CT combined with high-resolution MRI to evaluate drug-induced repair, and artificial intelligence-assisted preoperative planning. (3) International keyword hotspots have centered on “osteonecrosis of the femoral head/avascular necrosis/joint replacement” as a classic core (average year: 2021), while recent trends have expanded into cutting-edge areas such as “collapse biomechanics, finite element analysis, minimally invasive hip preservation (arthroscopy), and COVID-19” (average years: 2022–2023). The emergence intensity of keywords such as artificial intelligence, deep learning, and machine learning has continued to rise (2020–2026); internationally highly cited publications focus on original work such as the development of the new ARCO CT classification system, the development of deep learning models, and the identification of imaging biomarkers (bone resorption volume ratio, dynamic changes in cortical bone). (4) The data above indicate that research hotspots in China have shifted from traditional imaging diagnosis to the quantitative assessment of diagnostic performance, with certain results in applications such as artificial intelligence-assisted preoperative planning. However, compared with foreign institutions, research in China still lags significantly in terms of original theoretical and methodological contributions, such as the development of new imaging classification systems, the development of original deep learning diagnostic models, and the identification of imaging biomarkers, and has yet to establish a new research paradigm. In contrast, research hotspots at foreign institutions are guided by the “construction of new systems,” leading to breakthroughs in cutting-edge areas such as the ARCO CT classification, deep learning diagnostic models, and ‌coronavirus disease 2019‌-related osteonecrosis. (5) Recommendations for future development should focus on the deep integration of artificial intelligence and imageomics, the integration of multimodal imaging and biomechanical modeling, and the establishment of a multicenter standardized validation system, thereby driving a paradigm shift from comparative efficacy studies to original research.
