Prospects and problems of stem cells in the treatment of neurodegenerative diseases

Abstract:

BACKGROUND: Although the incidence of neurodegenerative diseases in various populations is high and has an upward trend, so far there is no effective treatment method. So it is a challenge for scientific community to seek effective treatment method for the treatment of neurodegenerative diseases.
OBJECTIVE: To analyze and review the research progress and hot issues of stem cells for the treatment of neurodegenerative diseases.
METHODS: The basic and clinical researches on stem cells for the treatment of neurodegenerative diseases were overviewed, including Alzheimer’s disease, Parkinson’s disease, stroke, Huntington’s disease, retinitis pigmentosa, amyotrophic lateral sclerosis and epilepsy, in order to investigate the feasibility, advantages and problems of stem cells in the treatment of neurodegenerative diseases.
RESULTS AND CONCLUSION: Most of the studies on stem cells in the treatment of neurodegenerative diseases are in the stage of experimental animal models, and there is no valid evidence to support stem cell therapy. There is no exact clinical assurance and standards on the safety and efficacy of stem cells for the treatment of neurodegenerative diseases, and the clinical efficacy and adverse reactions need to be further verified with large-sample long-term clinical trials.

2.1 帕金森病帕金森病是一种以纹状体多巴胺能神经元功能进行性丧失为特征的慢性神经退行性疾病，在临床上以震颤、肌肉强直、运动减少和姿势平衡障碍作为主要症状。要想治疗帕金森最好就是将受损的细胞进行修复，而能将受损细胞修复的只有干细胞。干细胞治疗帕金森病原理是根据细胞是多能细胞，终身具有自我更新能力，可以被诱导分化为各种类型的成熟神经细胞[6]。它是神经系统形成和发育的源泉，主要功能是作为一种后续储备，参与神经系统损伤的修复或正常神经细胞凋亡的更新。药物治疗帕金森病效果有限，采用脑立体定向技术损毁丘脑和苍白球，有残疾和生命危险，此外治疗后复发也是影响疗效的主要原因[7]。而干细胞治疗可以从根本上解决帕金森病的问题，利用干细胞治疗帕金森病具有独特的优势[8-9]：①治疗效果好，利用干细胞具有自我更新能力，在病变部位不断增殖，重建神经网络，产生神经营养因子或神经保护因子，从而抑制神经变性或促进神经再生利用。②治疗方式安全可靠，干细胞治疗没有任何毒副作用，具有更高的治疗安全性。③无排异反应，干细胞治疗直接作用于脑部神经系统，有效的躲开了血脑屏障，并且干细胞本身具有低免疫力，治疗后不会造成免疫系统排斥的危险。现在针对治疗帕金森的问题各国专家均提出不同种类干细胞治疗的方案。 Fu等[10]将未诱导和经过诱导的人脐带间充质干细胞移植到6-羟基多巴诱发的帕金森病大鼠模型的纹状体内，发现表达人特异性核抗原的酪氨酸羟化酶阳性细胞可在移植部位存活达4个月以上，能向移植部位的头、尾两侧迁移约1.4 mm，苯丙胺诱发的大鼠转圈行为也得到显著改善。这提示人脐带间充质干细胞来源的酪氨酸羟化酶阳性细胞有望成为治疗帕金森病的理想细胞。高华等[11]为探讨脂肪基质干细胞诱导分化为巢蛋白阳性的神经干细胞后移植治疗帕金森病大鼠模型的疗效及作用机制。将大鼠随机分为3组，16只大鼠为正常对照组、22只帕金森病模型大鼠为生理盐水组、22只帕金森病模型大鼠为干细胞移植组。5-溴脱氧尿嘧啶标记神经干细胞，生理盐水 12 μL分别注入模型鼠右侧纹状体，在干预后2，4，8周分别观察行为学变化；脑冰冻切片免疫荧光染色检测双标细胞；8周时，免疫组织化学法检测脑纹状体区酪氨酸羟化酶数量变化，高效液相色谱法检测脑内多巴胺的含量。结果发现脂肪基质干细胞可定向诱导表达巢蛋白的神经干细胞；细胞移植干预后帕金森病大鼠行为学得到明显改善，因此，认为脂肪源性的神经干细胞移植治疗帕金森病大鼠可以有效改善行为学症状。Wernig等[12]从大鼠诱导多能干细胞的体外培养中有效的分化为神经前体细胞，然后将它们移植到帕金森病大鼠脑的各个部位，干细胞迁移到不同脑区并分化为神经胶质细胞和神经元，结果诱导多能干细胞来源的神经元与受体大鼠原有的神经元形成突触，并且使帕金森病大鼠的症状得到明显改善。所有这些动物实验研究结果为帕金森病初期临床实验提供了理论依据。早在2004年，Oka等[13]报道了自体神经前体细胞移植治疗帕金森病患者，对于该种治疗方法的长期安全性以及治疗效果是个有益的尝试。2008年，Park等[14]从1例57岁男性患者的表皮成纤维细胞中得到帕金森病特异性人诱导多能干细胞，疾病特异性人诱导多能干细胞可用于病因学、毒理学研究和药物筛选等。2009年，Soldner等[15]成功的从7例帕金森病患者的成体细胞中诱导得到人诱导多能干细胞，并将帕金森病患者的成纤维细胞与特发性帕金森病可以有效的重新编程，随后分化成多巴胺能神经元。更值得注意的事，在分化效率的检测中，诱导多能干细胞能够保持多向分化潜能而且与人胚胎干细胞特性很接近，因此，这些细胞对于建立人类的疾病模型有着更加重要的意义。 2.2 阿尔茨海默病阿尔茨海默病又叫老年性痴呆，是慢性进行性中枢神经系统变性病导致的，是痴呆最常见的病因和最常见的老年期痴呆[16]。阿尔茨海默病以渐进性记忆障碍、认知功能障碍、人格改变以及语言障碍等神经精神症状为特征。阿尔茨海默病的治疗包括神经保护疗法、胆碱酯酶抑制剂、采用非药物干预和精神药理学药物减少行为障碍等。这些治疗方法主要是针对早期患者的，作用十分有限，主要是不同程度的减缓疾病的进展。而干细胞这种新的治疗方法，正在临床上开展试验治疗阿尔茨海默病，以最终确定干细胞的疗效[17]。目前许多大学和医疗设施在开展这个领域的工作。干细胞治疗阿尔茨海默病的技术细节可有不同，本质上通过移植干细胞来对神经组织进行修复和再生。干细胞为阿尔茨海默病神经系统的损伤修复和治疗提供了一个崭新的手段，具有广阔的应用前景。干细胞治疗阿尔茨海默病的目的是修复和替代受损的神经细胞，重建细胞环路和细胞功能。模式动物已被有效的用来研究阿尔茨海默病，虽然它不能复制这种疾病的各个方面，但是有助研究阿尔茨海默病。邬伟等[18]为探讨骨髓间充质干细胞对阿尔茨海默病大鼠空间学习记忆能力的影响，将雄性Wistar大鼠45只，随机分为假手术对照组、双侧穹窿海马伞切断模型组和骨髓间充质干细胞治疗组。采用Morris水迷宫进行学习、记忆能力的测定，以大鼠逃避潜伏期代表学习记忆能力，测定方法同文献所述[19]，逃避潜伏期越短，大鼠学习记忆能力越强。结果发现与对照组相比，模型组大鼠逃避潜伏期明显延长(P < 0.05)，与模型组相比，骨髓间充质干细胞治疗组大鼠逃避潜伏期缩短(P < 0.05)。因此，认为骨髓间充质干细胞治疗能够改善阿尔茨海默病大鼠空间学习记忆能力。Sugaya等[20-21]研究用神经干细胞和间充质干细胞联合小分子复合移植治疗阿尔茨海默病大鼠，使模型鼠的认知功能得到改善，因此，认为通过成体神经干细胞或间充质干细胞治疗使年老的动物在认知功能上和神经生成上得到较大的改善。目前，干细胞治疗阿尔茨海默病受到多方面的限制，需要进一步深入的研究，例如阿尔茨海默病的大脑本身的结构功能改变、内环境的改变等[22]。此外，干细胞治疗阿尔茨海默病的研究多限于动物模型，真正用于人的阿尔茨海默病临床治疗之前，仍然面临很多难题，比如干细胞的移植剂量如何确定、如何选择干细胞的移植位点、如何评测干细胞的长期疗效、潜在不良反应及安全性等[23]。 2.3 视网膜变性疾病视网膜变性疾病是一组由于视网膜感光细胞变性凋亡所导致的严重的致盲性眼病，在哺乳动物视觉系统的变性性疾病中，视网膜内特殊细胞群的死亡往往导致视力损害，甚至失明[24]。因为这些致盲性的视网膜变性疾病都存在进行性的视网膜细胞丢失，常规的药物治疗缺乏疗效，所以理想的治疗方法是替代这些丢失的细胞。通过干细胞移植，使其整合入视网膜各层并且分化为目标细胞，以重建视网膜功能。随着干细胞研究的推广，越来越多的研究人员已经认识到干细胞作为一种细胞供体用于治疗视网膜变性疾病的潜能。大量的动物实验证实了干细胞移植到视网膜下腔或者玻璃体内后能够在宿主视网膜内迁移和分化为多种神经元和神经胶质细胞，并且能够发出突触，建立广泛的细胞连接，形成有功能的网络结构。Lund等[25]分别将人脐带组织细胞、胎盘细胞和骨髓间充质干细胞移植到视网膜变性疾病大鼠的视网膜下间隙，并以皮肤成纤维细胞作为对照。通过测定视网膜电流图、空间视敏度和亮度阈，发现脐带组织细胞和骨髓间充质干细胞均能显著减少视网膜退行性变，而且前者较后者对光感受器的修复作用更加显著。究其原因可能与脑源性神经营养因子和白细胞介素6的分泌有关，这无疑为视网膜变性疾病的治疗提供了新思路。张良等[26]发现视黄酸可以诱导大部分细胞成为神经细胞，在视黄酸和视网膜细胞共培养诱导条件下，可以获得更为纯化的形态一致的神经样细胞，部分诱导细胞表达视网膜细胞的特性，这给今后胚胎干细胞在视网膜中移植以治疗视网膜变性疾病中提供了有效参考。张杰等[27]证实了骨髓间充质干细胞能够在视网膜下存活并能与原视网膜结构相融合，尚不能判断形成的视网膜样结构是否具有神经细胞的特性，以及这种结构是否能够建立正确的神经传导。但是如能证明新形成的视网膜具有感光功能，骨髓间充质干细胞将成为视网膜移植高纯度、具有发育潜能的视细胞供体，在视网膜变性疾病治疗中将有广泛的应用潜力。在干细胞治疗视网膜变性疾病研究的数10年历史中，经动物实验证实可供进行移植的供体干细胞种类较多，而且随着研究的不断进步，还会有新的干细胞被发现[28]。干细胞在体外培养、增殖分化和体内移植方面的研究为视网膜变性疾病的治疗带来了希望，但目前面临着巨大的挑战是如何调控、触发干细胞分化，形成所需的供体细胞，使其能在移植后向光感受器细胞转化，与宿主视网膜整合，建立突触联系而修复视功能，同时避免排斥反应，避免伦理方面等问题，这些都需要基础研究的进一步深入研究。 2.4 亨廷顿病亨廷顿病也称舞蹈病，是常染色体显性遗传的缓慢进行性的变性病，临床上以舞蹈，精神障碍和痴呆为主要表现。其病理变化主要在大脑皮质和纹状体的尾状核。在尾状核和壳核有广泛小神经元缺失和胶质增生[29-30]。大脑皮质突出的变化为皮质萎缩，皮质尤其是额叶的第3和第4层神经细胞缺失明显。疾病晚期，其他神经元亦遭破坏。在生化方面有γ-氨基丁酸减少，其合成酶、谷氨酸脱羧酶活力减低。纹状体内乙酰胆碱和胆碱乙酰转移酶活力减退，但多巴胺能系统无影响。与大多数神经变性疾病一样，亨廷顿病缺乏特异性的治疗方法，主要采用对症治疗[31]。尽管尚无十分有效的药物可以延缓亨廷顿病的进展，但舞蹈样动作、精神障碍等常见症状通过合理的药物治疗均可获得不同程度的改善，而且还可以提高患者的生活质量并防止并发症发生。因此，在现阶段缺乏有效治疗方法的情况下，应重视对症治疗。目前，干细胞移植治疗又分为神经干细胞移植、胚胎干细胞移植和骨髓间充质干细胞移植，前者治疗亨廷顿病的研究最多，然而细胞移植治疗亨廷顿病尚处于探索之中，同基因治疗一样任重而道远。 2012年3月16日，复旦大学上海医学院解剖与组织胚胎学系青年教师马丽香在长江学者、复旦大学特聘教授张素春博士指导下，在学术期刊《细胞》的子刊《细胞-干细胞》杂志上发表研究成果[32]。研究发现，亨廷顿病产生原因是由于γ-氨基丁酸能神经细胞逐步退化后引起神经“环路”紊乱，进而导致患者出现运动功能障碍和认知能力逐渐丧失等一系列症状。于是，张素春教授研究团队把研究重点放在如何使人胚胎干细胞“定向诱导分化”为γ-氨基丁酸能神经细胞上，因为γ-氨基丁酸能神经细胞是哺乳动物中枢神经系统中重要的抑制性神经递质，对机体的多种功能有重要调节作用，如果脑内大量的γ-氨基丁酸能神经细胞死亡，就会产生“舞蹈样”动作。经努力，马丽香终于找到了一种可以从人胚胎干细胞中获得大量γ-氨基丁酸能神经细胞的有效方法，接下来，他们将获得的γ-氨基丁酸能神经细胞移植到患有亨廷顿病的模型鼠脑内，进一步研究移植细胞与内源细胞的整合情况。令他们吃惊的是，模型鼠的运动功能障碍有了明显减缓和改善，此证明被移植的细胞不仅能与内源细胞整合，而且还能有效地修复脑内损伤的“环路”。因为这一环路对协调“运动功能”非常重要，是必须的，而γ-氨基丁酸能神经细胞正是通过这一环路与远处细胞形成突触联系而起作用的。由于亨廷顿患者的这条环路遭到破坏，故出现上述许多不可逆症状。此前，脑神经研究领域的神经科学家都认为成人脑神经环路一旦被破坏很难修复，通过细胞移植来重建环路也是不可能的，但张素春教授研究团队的研究成果证明，移植的γ-氨基丁酸能神经细胞不仅能够重新建立这条至关重要的环路，而且还能产生正确的神经递质，它使干细胞移植治疗亨廷顿病成为可能。 2.5 肌萎缩性侧索硬化症肌萎缩侧索硬化是一种进行性神经变性疾病，由于上、下运动神经元变性导致延髓、四肢、躯干、胸部及腹部肌肉逐渐无力和萎缩，多因呼吸肌麻痹而死亡[33]。目前尚无有效的治疗方法，最新的研究表明，组织特异性干细胞具有分化成其他细胞或组织的潜能，可以分化为运动神经元，这为肌萎缩侧索硬化的治疗开创了更广泛的空间[34-35]。治疗肌萎缩侧索硬化的干细胞种类如下：胚胎干细胞：是指由胚胎内细胞或原始生殖细胞经体外抑制培养而筛选出的细胞。胚胎干细胞是一种高度未分化细胞。它具有发育的全能性，能分化出成体动物的所有组织和器官。但是人胚胎干细胞应用于临床面临伦理及形成畸胎瘤等复杂因素[36]。神经干细胞：指来源于神经系统, 具有自我更新能力。在特定条件诱导下能够向神经元、少突胶质细胞和星形胶质细胞分化的潜能[37]。骨髓间充质干细胞：具有多向分化潜能，能分化成神经元、神经胶质细胞。因骨髓间充质干细胞具有获取简便，对供体损伤小，增殖力强，来源广泛，具有迁移至组织损伤部位的能力；很强的免疫抑制特点，可自体移植，也可异体移植等优点[38]，骨髓间充质干细胞向神经细胞的分化为肌萎缩侧索硬化的治疗带来新的希望[39]。脐带血非造血干细胞：具有多向分化潜能，其在体外条件培养下可以分化为具有神经细胞和神经胶质细胞特征的细胞，植入到大鼠脑内可分化成胶质细胞或神经元[40]。诱导产生多能性干细胞：将人类体细胞诱导成为多能性干细胞的研究成果在生命科学领域引起了一次轰动，分化的细胞可以通过少数几个基因的外源导入而被重编程到具有多能性的状态，其意义在于可以将患者的体细胞重编程为多能干细胞，再分化为特异性的运动神经元[41]。到目前为止，虽然动物研究和部分临床试验证明干细胞对神经变性疾病的治疗非常有潜力，但是，干细胞治疗在临床广泛应用之前还有很长的一段路要走。到目前为止，人们对干细胞的了解仍存在许多盲区，还有许多问题需要解决。对肌萎缩侧索硬化病因研究的更多了解能为干细胞治疗提供更多帮助。随着基因工程、胚胎工程、细胞工程等各种生物技术的快速发展，干细胞研究领域向深度和广度不断扩展，人们对干细胞的了解也将更加全面[42-43]。"
