局部应用布比卡因脂质体降低脊柱手术后疼痛及并发症的Meta分析

Local administration of liposomal bupivacaine reduces postoperative pain and complications in spinal surgery: a meta-analysis

布比卡因脂质体:是一种含有布比卡因的多囊泡脂质体制剂,利用特有的DepoFoam技术将布比卡因封装在多囊脂质体中,其内部包含许多水性囊泡,随着每个囊泡的不断破裂,布比卡因从破裂的囊泡中释放,由于不同囊泡破裂的时间不同,布比卡因得以缓慢释放,从而能够提供安全浓度的布比卡因并实现72 h以上的镇痛作用。
多模式镇痛:是通过联合不同作用机制的镇痛药物和多种镇痛方法,作用于疼痛传导通路的不同靶点,发挥镇痛的相加或协同作用,减少外周和中枢敏感化,而获得最佳镇痛效果,同时能够减少单种镇痛药剂量,减少不良反应。

摘要
目的:脊柱手术围术期疼痛管理一直是医疗团队十分关切的临床问题。目前,在多模式镇痛理念下,在脊柱手术中采用布比卡因脂质体局部浸润麻醉取得了一定的进展。此次荟萃分析旨在评价脊柱手术中局部应用布比卡因脂质体是否能有效降低术后疼痛、阿片类药物消耗量及相关并发症的发生率。
方法:严格按照 Cochrane手册提供的方法学指导进行研究,研究方案已在 PROSPERO注册。由2名研究人员独立在中国知网、万方数据库、维普、PubMed、Web of Science、Embase、Cochrane Library和Google Scholar进行文献检索,收集2015年1月至2024年12月符合纳入标准的试验研究,并完成数据的收集和整理。纳入研究的偏倚风险或文献质量由2名评估员独立评估。采用STATA 软件对数据进行统计分析。
结果:①共纳入13项研究包括2 962例患者,纳入随机对照试验的偏倚风险被认为是低到中等,队列研究的质量评分均被评为高质量;②结果表明,布比卡因脂质体组的术后阿片类药物总消耗量比对照组更低,差异有显著性意义[SMD=-0.36,95%CI(-0.71,-0.02)],但敏感性分析显示该结果的稳健性不高;③此外,布比卡因脂质体组各随访点的疼痛得分、阿片类药物消耗量以及治疗相关并发症发生率与对照组相比差异均无显著性意义 (P > 0.05)。
结论:脊柱手术中局部应用布比卡因脂质体不会增加与治疗相关的并发症,且能够有效减少阿片类药物的总消耗量;但布比卡因脂质体在改善脊柱术后疼痛方面并未表现出明显的优势。

Abstract: OBJECTIVE: The purpose of this meta-analysis was to assess whether the local application of liposomal bupivacaine in spinal surgery effectively reduces postoperative pain, opioid consumption, and the incidence of related complications.
METHODS: The study was conducted strictly in accordance to the methodological guidelines proposed by Cochrane Handbook. The protocol of the present work was registered with PROSPERO. Two researchers independently conducted literature searches in electronic databases including CNKI, WanFang Data, VIP, PubMed, Web of Science, Embase, Cochrane Library, and Google Scholar, collecting eligible trials published between January 2015 and December 2024, followed by systematic data collection and organization. The risk of bias or the quality of each included study was evaluated by two assessors. Data were analyzed using STATA software.
RESULTS: (1) 13 studies involving 2 962 patients were included in the present work. The risk of bias of randomized controlled trials were rated as low to moderate, while the quality of the included cohort studies was allocated to high. (2) The results of this study revealed that total opioid consumption in the liposomal bupivacaine group was significantly lower than that in the control group [SMD=-0.36, 95% CI (-0.71, -0.02)]. However, the sensitivity analysis indicated that the robustness of this result was low. (3) Furthermore, the pain scores, opioid consumption and treatment-related complication rates at each follow-up point in the liposomal bupivacaine group were not significantly different from those in the control group (P > 0.05).
CONCLUSION: Local administration of liposomal bupivacaine in spinal surgery did not increase the risk of treatment-related complications and effectively reduced total opioid consumption. However, no enough evidence supported that liposomal bupivacaine could significantly improve pain intensity during the spinal surgery.
