全膝关节置换对线技术:最新研究热点文献可视化分析

对线技术:是全膝关节置换的核心技术体系,旨在通过三维空间内的精准截骨规划、假体个性化定位及动态软组织平衡,协同实现两大目标:①生物力学优化:重建下肢机械轴,均匀分散关节载荷以延长假体寿命;②解剖功能还原:基于患者固有解剖特征(如骨性形态、韧带张力)与功能需求,恢复自然关节运动学特性。其技术精髓在于突破传统机械对线的标准化束缚,转向个体化适配方案(如运动学对线、功能性对线),通过毫米级精度控制截骨角度、假体旋转对位及间隙平衡,最终达成术后关节的长期稳定性、无痛运动功能与远期生存率提升。 
文献计量学:是指用数学和统计学的方法定量分析知识载体的交叉科学,是集数学、统计学、文献学为一体,注重量化的综合性知识体系。文献计量学方法是客观定量的,它们避免了与系统综述中的样本选择有关的偏差,也是反映某一领域的研究趋势和预测研究热点的重要手段,其优势在于基于多学科对特定研究领域进行定性和定量评价。

摘要
背景:全膝关节置换部分患者对术后疗效并不满意,其中活动受限、关节失稳及残余疼痛是主要原因。为解决上述问题,不同的截骨方式、对线技术相关理念相继提出并应用于临床,但是何种方式最佳尚未达成共识。
目的:全面分析全球全膝关节置换过程中不同对线技术的应用趋势,并基于文献计量学和可视化分析确定对线技术研究热点,为医生进行临床决策时提供参考。
方法:从Web of Science数据库(https://www.webofscience.com,开发机构: 美国科学信息研究所)中检索了2015-01-01/2024-12-31共10年与全膝关节置换对线技术相关的出版物(论著和综述),提取并详细分析了国家、机构、出版年份、作者、期刊、每篇文章的平均引用率、H指数、标题、出版物的关键词以及被引用次数最多的25篇文章。利用VOSviewer、Citespace软件分析关键词的共现结果,预测全膝关节置换对线技术的研究热点。
结果与结论:①共计检索出866篇关于全膝关节置换对线技术的文献,论著792篇,综述74篇;②美国发文量最多,加利福尼亚大学是发文量最多的机构,而Howell, Stephen M是发文量最多的作者;《Knee Surgery Sports Traumatology Arthroscopy》期刊发文量与引用量最多;③常见的关键词是全膝关节置换、对齐技术、关节成形术、临床结果、生存率;关键词与参考文献爆发表明,运动力学对线、机器人导航是未来的研究热点;④说明全膝关节置换对线技术相关论文发表量正在逐年增加,美国现在是全膝关节置换对线技术研究发文量最多的国家,也是该领域的最大贡献者,综合实力英国紧随其后;越来越精准的工具正在被设计并应用于全膝关节置换截骨、对线技术,包括计算机辅助导航、个性化截骨工具以实现对齐目标,但目前仍然没有最佳解决方案;未来“机器人辅助导航” “患者满意度” “远期生存率” “适应证范围” “个性化截骨对线”可能是全膝关节置换对线技术研究的前沿热点。

Abstract: BACKGROUND: Some patients report dissatisfaction after total knee arthroplasty due to persistent issues like limited mobility, joint instability, and residual pain. To address these challenges, various osteotomy methods and alignment strategies have been proposed and clinically implemented. However, the optimal approach remains debated, with no clear consensus.  
OBJECTIVE: To comprehensively analyze global trends in alignment techniques for total knee arthroplasty and identify research hotspots through bibliometric and visual analysis, offering evidence-based insights to guide clinical decision-making.  
METHODS: We retrieved publications (articles and reviews) on total knee arthroplasty alignment techniques published between January 1, 2015 and December 31, 2024, from the Web of Science database (https://www.webofscience.com, developed by the Institute for Scientific Information, USA). Data on countries, institutions, publication years, authors, journals, mean citation rates, H-index, titles, keywords, and the top 25 most-cited articles were extracted and analyzed. Keyword co-occurrence was assessed using VOSviewer and CiteSpace to predict research trends of alignment technology in total knee arthroplasty.  
RESULTS AND CONCLUSION: (1) Totally 866 retrieved articles (792 research articles, 74 reviews) were found. (2) United States accounted for the highest number of publications. The University of California was the most prolific institution, and Stephen M. Howell was the most published author. Knee Surgery Sports Traumatology Arthroscopy had the highest publication and citation counts. (3) Frequent keywords included: total knee arthroplasty, alignment techniques, arthroplasty, clinical outcomes, and survival rate. Keyword and reference analysis highlighted kinematic alignment and robotic navigation as emerging research priorities. (4) It is concluded that publications on total knee arthroplasty alignment techniques continue to grow annually. The United States dominates research output and is now the country with the most articles published on total knee arthroplasty alignment technology and the largest contributor in this field, followed by the United Kingdom in overall contributions. Advanced tools like computer-assisted navigation and patient-specific osteotomy instruments are increasingly used to achieve alignment goals, though no universally optimal solution has been established to date. In the future, "robot-assisted navigation," "patient satisfaction," "long-term survival rate," "scope of indications," and "personalized osteotomy alignment" may be hot topics in the research of total knee arthroplasty alignment technology.
