骨水泥型人工双极股骨头置换治疗骨质疏松性股骨颈骨折

Cemented bipolar hemiarthroplasty for osteoporotic femoral neck fracture

摘要:

背景:老年骨质疏松性特殊体质及对置换耐受性差的特点,使一旦发生股骨颈骨折存在着合并症多,并发症高的问题。
目的:观察骨水泥型人工双极股骨头置换治疗股骨颈骨折的临床疗效。
方法:选择2006-05/2009-03赣州市人民医院骨科收治的骨质疏松性股骨颈骨折患者39例,均采用骨水泥型人工双极股骨头进行置换。
结果:随访39例10~34个月。置换后6,12个月Harris评分分别为(92.5±6.8),(93.2±5.6)分,较置换前(42.5±9.6)分明显提高(P < 0.01)。影像显示所有股骨假体位置无移动,未见松动或下沉。末次随访未见可确诊的无菌性松动影像表现或要求翻修,亦无骨盆局限性骨溶解。提示采用骨水泥型人工双极股骨头置换治疗老年性骨质疏松性股骨颈骨折的临床效果满意。

Abstract:

BACKGROUND: The elderly commonly have their special physical characteristics of osteoporosis and poor tolerance to surgery. There are the higher complications and more mortality with femoral neck fracture in the elderly.
OBJECTIVE: To investigate the cemented bipolar hemiarthroplasty effect on osteoporotic femoral neck fractures in the elderly.
METHODS: Thirty-nine senior patients with osteoporotic femoral neck fractures admitted to the Department of Orthopaedics, People’s Hospital of Ganzhou City from May 2006 to March 2009, were selected and all the patients received cemented bipolar hemiarthroplasty.
RESULTS AND CONCLUSION: All the 39 cases were followed up for 10 to 34 months. The Harris hip function scores were 92.5±6.8 and 93.2±5.6 respectively at 6 and 12 months after cemented bipolar hemiarthroplasty, which were significantly higher than that (42.5±9.8) preoperatively (P < 0.01). Imaging examination showed that the location of all femoral prostheses was unchanged, no loosing or sinking. The final follow-up showed no aseptic loosening could be diagnosed with imaging findings or no requirement for renovation was found as well as non-limited pelvic osteolysis. The cemented bipolar hemiarthroplasty can reduce complications and lower mortality rates for the elderly with osteoporotic femoral neck fractures in a better way.
