2.5 影响人工全髋关节置换后隐性失血的单因素分析 结果显示,高隐性失血组的年龄、高血压比例、糖尿病比例、骨质疏松比例、手术时间、总失血量、术中失血量、术后引流量及血红蛋白下降量均高于低隐性失血组,差异有显著性意义(P < 0.05);低隐性失血组的骨水泥型假体构成比、前方入路构成比及2-3级髋关节间隙高于高隐性失血组,差异有显著性意义(P < 0.05);两组在性别比例、体质量指数、慢性支气管炎比例、侧别比例、麻醉方式及髋臼杯直径方面比较差异无显著性意义(P > 0.05)。具体结果见表4。 2.6 术后隐性失血多因素Logistic回归模型分析 将高低隐性失血(低=0,高=1)作为因变量,将单因素分析选出的因素作为自变量并进行赋值,见表5。 多因素Logistic回归分析的结果显示,年龄、糖尿病、骨质疏松、假体类型、手术入路、髋关节间隙、总失血量、术中出血量、术后引流量及血红蛋白下降量是人工全髋关节置换后高隐性失血的危险因素(P < 0.05),见表6。 Logistic回归模型:P=ea/(1+ea),a=-2.153+0.730×年龄赋值+0.746×糖尿病赋值+1.162×骨质疏松赋值+1.106×假体类型赋值+1.258×手术入路赋值+1.246×髋关节间隙赋值+1.110×总失血量赋值+1.133×术中出血量赋值+ 1.138×术后引流量赋值+1.164×血红蛋白下降量赋值,各独立危险因素赋值见表5。式中e为指数函数,P为人工全髋关节置换后高隐性失血的概率。"
