T lymphocytes and immune tolerance of liver transplantation

Abstract:

BACKGROUND: Pathogenesis of liver graft rejection is mainly the immune response mediated by T cells. Low-molecular-weight proteins with high activity and multiple functions that can regulate cell functions, i.e. cytokines, exert an important role in organ-graft-rejection and immune tolerance.

OBJECTIVE: To review the relation and current research of T lymphocytes and immune tolerance of liver transplantation.

METHODS: A computer-based online search was conducted by the first author in Medline and VIP Database from January 1995 to June 2013 for articles concerning immune state after liver transplantation, T lymphocytes and its cytokines. The key words were “liver transplantation, immune tolerance, rejection, T lymphocytes”. Duplicated studies were excluded. Full texts were searched. A total of 57 related articles were accepted. Nine articles concerned the background research of T lymphocytes with immune status after transplantation. Ten articles addressed the role of regulatory T cells in transplantation immunology. Seventeen articles were about the relation of T lymphocytes and immune tolerance after transplantation. Twenty-one articles were on the research prospects of T lymphocytes and immune tolerance after transplantation.
RESULTS AND CONCLUSION: T lymphocytes are important cells to regulate the immune response. Immunologic rejection or immune tolerance depended on subsets and functions of T lymphocyte in the immune system after receiving allogeneic liver transplantation. We can induce host immune tolerance through inhibition or induction of some features of T lymphocytes.

2.1 免疫耐受机制 2.1.1 中枢耐受 T细胞主要在胸腺内发育成熟，在此发育过程中，T细胞经历了严格的筛选过程。其中Ｔ细胞受体是移植抗原识别的关键结构。T细胞受体结构的多样性决定了T细胞在离开胸腺进入外周淋巴细胞之前必须经过严格的选择。某些T细胞受体对自身抗原有很强的反应性，这些T细胞必须被清除出机体的免疫系统，否则会导致自身免疫性疾病。这一选择过程被称为阴性选择；另外，T细胞对抗原的识别具有“MHC限制性”，因此只有那些T细胞受体能与自身MHC相互作用的T细胞才能被选择出来，继续在胸腺内发育成熟，这一过程称为“阳性选择”。研究表明，胸腺中约5%的原始T细胞，其T细胞受体能识别自身MHC-Ⅰ类或Ⅱ类抗原，这群细胞便得到了继续发育的信号，能够在胸腺中继续发育成熟，而那些T细胞受体与自身MHC无亲和力的T细胞无法得到继续发育成熟的信号，即发生了调亡或表现为无能。通过阳性选择和阴性选择保留下来的T细胞，正常情况下对自体的MHC不起反应，但随时保持对外来抗原及自身突变细胞具有免疫应答能力[8]。 2.1.2 外周耐受外周耐受机制实际上是一种胸腺外淋巴组织的耐受机制，外周T淋巴细胞可通过无能、调节作用、克隆消除、衰竭、免疫忽视和诱导细胞凋亡等手段，使自身反应性淋巴细胞受到严格控制。这些理论的一个重要结果是机体对组织特异性表达的自身分子的耐受，主要是通过外周耐受来实现的，并依赖于抗原递呈细胞，尤其是未成熟树突状细胞对这些自身分子的捕获[2]。树突状细胞是最主要的抗原递呈细胞，在抗原递呈、激活T细胞反应中发挥重要的作用[9]。相反不成熟树突状细胞在免疫耐受形成中发挥着重要作用。胸腺淋巴样树突状细胞在淋巴结的T细胞区表达高水平的MHC-Ⅱ类分子/自身抗原肽。胸腺淋巴样树突状细胞在诱导T细胞增殖的同时并不产生白细胞介素2、白细胞介素3、γ-干扰素、GM-CSF等细胞因子。在外周胸腺淋巴样树突状细胞亦通过Fas介导的凋亡来清除CD4+T细胞。 T细胞在经过MHC/抗原肽-T细胞受体特异性结合后，既不能增殖、亦无白细胞介素2等细胞因子产生的一种状态称为T细胞无能。T细胞被抗原激活必须有3个信号的参与：MHC限制性的抗原识别为第一信号；共刺激信号即第二信号，主要为一些细胞表面分子，包括CD28:B7.1/B7.2、CD40:CD154、LFA-1:ICAM-1等；白细胞介素2通过自分泌或旁分泌的方式提供了第三信号。T细胞无能可通过人为阻断共刺激信号而诱导，无能T细胞其白细胞介素2的基因表达被部分或完全阻断。这种无能的T细胞在以后接触由成熟抗原呈递细胞(antigen presenting cell)递呈的特异性抗原时不能被激活，无能状态可持续长的时间。在大多数情况下，在体内阻断共刺激通路而致无能的T细胞对外加的白细胞介素2并无反应。部份T细胞分泌的可溶性细胞因子通过调节作用来维持外周免疫耐受，包括抑制性T细胞、自然抑制性细胞、抗独特型网络以及白细胞介素10、转化生长因子β等细胞因子。抑制性T细胞识别抗原后，抑制效应T细胞的活化，或者通过分泌抑制性因子来阻碍效应T细胞的激活。大量的自身或外来抗原长期存在，便可导致大量的反应性T细胞激活继而发生衰竭。 2.2 T淋巴细胞在肝移植免疫中发挥的作用 T淋巴细胞按其功能可分为CD4+辅助性T细胞(Th)、CD8+杀伤性T细胞(细胞毒性T细胞)、抑制性T细胞。Th细胞可分为Th0、Th1、Th2、Th3和Th17。其中Th1、Th17主要与免疫排斥反应相关，而Th2、Th3则与诱导免疫耐受相关。目前研究比较多的为Th1、Th2。Th1主要分泌细胞因子白细胞介素2、γ-干扰素，与急性排斥反应相关。主要功能是促进细胞免疫应答，与迟发型超敏反应T细胞和细胞毒性T细胞(CTL)的增殖、分化、成熟有关，也可激活巨噬细胞和天然杀伤细胞，特异性地杀伤移植物抗原，启动排斥反应，并促进Th0向Th1细胞分化。Th2主要分泌细胞因子白细胞介素4、白细胞介素10，诱导免疫耐受。主要通过抑制活化的T细胞产生白细胞介素2；抑制自然杀伤细胞产生γ-干扰素；抑制抗原提呈细胞的呈递作用；促进调节性T细胞的发育；下调MHCⅡ类分子的表达；抑制单核巨噬细胞的活化。有研究报道移植物长期存活的受体中表达高水平白细胞介素4和白细胞介素10，而白细胞介素2降至不能检测水平；与此相反，发生移植物急性排斥的患者显示白细胞介素2和γ-干扰素的高表达，而白细胞介素4和白细胞介素10则降到不能检测水平[10-12]。正常情况下Th1/Th2保持平衡状态，但在肝移植后早期，Th1较移植前轻度下降，Th2逐渐升高，使得Th1/Th2向Th2漂移，从而诱导免疫耐受。因此认为Th1/Th2的动态平衡对免疫耐受起着重要的作用，Th1向Th2细胞发生免疫偏离是移植耐受的机制之一[13-15]。 2.3 调节性T细胞在移植免疫耐受中的作用调节性T细胞是另一个独立的功能性T细胞亚群，它具有特异性抑制自身反应性T细胞的功能，是参与维持外周耐受(胸腺后)的重要细胞。调节性T细胞主要是通过细胞与细胞间接触或产生免疫抑制细胞因子如白细胞介素10或转化生长因子β发挥抑制作用。这种细胞主要表达CD4CD25，还有重要的膜表面分子细胞毒性T细胞相关抗原4。细胞毒性T细胞相关抗原4在调节性T细胞的调节作用中起着重要的作用。越来越多的研究表明，CD4CD25 T细胞在移植后免疫耐受中起到关键作用[16]。Tapirdamaz等[17]发现，肝移植受体的细胞毒性T细胞相关抗原4基因存在+49A/+6230G等位基因风险时，移植后患者发生急性排斥反应的概率远高于正常患者，细胞毒性T细胞相关抗原4是调节性T细胞功能表达的重要分子，表明调节性T细胞功能与宿主对移植肝脏的免疫耐受有关。Demirkiran等[18]也发现在肝移植后发生急性排斥反应的患者，其体内循环中的CD4CD25调节性T细胞的数量较未发生排斥的患者要低的多，这表明提高受体调节性T细胞的数量有利于肝移植物的存活[19-25]。有动物实验发现，单次注射调节性T细胞能够在受者体内诱导长期的抗原特异性无反应，这种耐受效果可能与转移的调节性T细胞“教育”供者CD4+细胞发展为CD25+细胞有关[26]。文献报道调节性T细胞在移植耐受的诱导和维持过程中具有“连锁抑制性”和“传染性耐受”等特征[21]。“连锁抑制性”是指调节性T细胞对移植物某一抗原的免疫无应答，可诱导产生对移植物所有抗原的无应答，这一过程需要抗原呈递细胞的参与。当受者调节性T细胞和同种反应性T细胞识别同一抗原呈递细胞上递呈的不同的供者抗原肽时，调节性T细胞可通过连锁抑制诱导潜在的同种反应性T细胞产生调节活性[27]。由于“连锁抑制”被诱导的同种反应性T细胞作为二种“继发性”的调节性细胞，可以依次的诱导其他初始CD4 T细胞表现出调节性表型，从而使耐受状态持续，这种现象称为“传染性耐受”。 2.4 诱导肝移植后免疫耐受的基本策略及方法 2.4.1 短期免疫抑制剂的应用使用免疫抑制剂产生免疫耐受的基本原理是在改变受者免疫功能状态的前提下，控制同种异体抗原递呈的途径和形式，以促使同种反应性T细胞产生耐受。近年来研究者发现，外周T细胞被抗原或超抗原激活后会开始克隆扩增，发挥效应机制，而随后大部分活化的T细胞均会死亡，即所谓活化诱导的细胞死亡。研究发现活化诱导的细胞死亡与Fas/FasL介导的活化T细胞调亡有关。进一步研究发现，活化诱导的细胞死亡可通过免疫抑制药物(如OKT3、ATG)来诱导，亦可由某些抗代谢药及基因毒性药物(如霉酚酸酯、5-氟尿嘧啶等)来触发[28-29]。克隆清除机制主要影响已经经过克隆扩增的反应性T细胞，因为这些细胞对活化诱导的细胞死亡最为敏感。因此同种移植物能够度过移植物后早期的免疫攻击，它就有可能为宿主长期接受。然而现阶段常用的免疫抑制剂如环孢素A、他克莫司，在抑制T细胞对移植物免疫反应的同时，也阻断了活化诱导的细胞死亡这一重要的生理稳定机制，同时抑制了被认为与移植物长期接受密切相关的调节性T细胞。所以目前常用的免疫抑制方案实际上有可能阻断肝移植后免疫耐受的形成。在这样的背景下，免疫抑制剂使用方案的调整及短期应用逐渐引起人们的重视[30-33]。 2.4.2 基因治疗在移植免疫耐受中的应用随着现代分子生物学的迅猛发展，基因工程技术被广泛应用于免疫耐受的研究中。通过严格的转基因及基因敲除技术可以准确地知道某一分子在免疫耐受中的作用，从而用于诱导免疫耐受。由于树突状细胞在免疫耐受中起着关键的作用，可通过基因转导技术，使表达CTLA4Ig基因的树突状细胞其表面CD8分子的表达显著下降，并表现出诱导同种抗原特异性T细胞无反应及显著延长移植物在受体内的生存时间等作用。这样经过CTLA4Ig基因转导的树突状细胞递呈同种抗原由于缺乏第二信号可以致反应性T细胞克隆无能；同时在局部产生许多免疫抑制分子如CTLA4Ig，有助于诱导受者对移植物的耐受，并最大限度地减少全身性的免疫抑制作用。另外通过反转录技术，使树突状细胞前体细胞稳定的产生白细胞介素10及使细胞表面MHC和共刺激信号分子表达明显下降，从而达到致T细胞无能、选择性激活Th2细胞及诱导淋巴细胞调亡等机制产生受者T细胞的低反应性[34-37]。 2.4.3 单克隆抗体诱导免疫耐受移植后排斥反应主要是细胞介导的免疫反应，即在异体抗原刺激下T细胞的活化及致敏T细胞的克隆增殖。各种单克隆抗体就是通过阻断这一过程中的不同环节而发挥免疫抑制作用及诱导免疫耐受。但是由于免疫耐受的机制尚不十分明确，使得应用单克隆抗体诱导耐受的结果并不如人们预计的乐观。目前，尚无一种以单克隆抗体为主的治疗方案可以显示免疫耐受结果。一般情况下，单克隆抗体只是配合其他诱导耐受手段使用，起到抑制当前免疫反应，从而为免疫耐受的建立提供条件的作用。迄今为止，研究较多的单克隆抗体有抗白细胞介素2受体单抗，抗T(CD4、CD8)细胞亚群单抗，抗黏附分子单抗，以及阻断共刺激信号的单抗等等。这其中对CD4和CD8分子的功能控制研究较多。由于CD4、CD8分子为MHC-抗原肽/T细胞受体结合的辅助分子，可加强MHC-抗原肽/T细胞受体结合物的稳定性，促进T细胞激活。抗CD4或CD8单抗可覆盖CD4、CD8分子，阻断上述结合过程，从而引起T细胞的“免疫无反应性”[37]。 2.4.4 诱导T细胞无能而产生免疫耐受通过阻断共刺激信号通路可致T细胞无能，这样的T细胞对抗原再刺激呈现无反应性。这种无能具有不稳定性，防止这种无能的逆转尚需其他特异信号[38]。此种诱导耐受的方法是目前研究较活跃的领域，尤其是抑制B7-CD28共刺激通路。抑制B7-CD28第二信号可应用抗CD28单克隆抗体。另外，细胞毒性T细胞相关抗原4融合蛋白亦为抑制B7-CD28路径的有效试剂。近年来研究发现，CD40-CD40L的结合较B7-CD28的结合是更早的分子事件，应用抗CD40L单克隆抗体阻断此第二信号通路，可以使小动物实体器官移植的存活时间明显延长。因此联合阻断B7-CD28及CD40-CD40L通路可能为更为有效的诱导免疫耐受[39]。"
