Multiple effects of ultrasoundcardiogram on monitoring of heart transplantation

Abstract

Abstract:

BACKGROUND: Acute rejection is a key factor for long survival of transplanted heart. Therefore, discovering and controlling acute rejections in time is very important in clinical practice.

OBJECTIVE: To investigate the clinical significance of ultrasoundcardiogram after heart transplantation.

METHODS: Recently published articles concerning clinical and animal research on ultrasoundcardiogram following heart transplantation were searched. Then, we analyzed the features of ultrasoundcardiogram of transplanted heart, structural and functional changes of donor hearts in the recipients. Relevant markers were measured and compared to evaluate the effect of ultrasoundcardiogram against acute rejections after heart transplantation.

RESULTS AND CONCLUSION: Ultrasoundcardiogram can invasively monitor acute rejections after heart transplantation in time, and also can provide important references for pre-transplantation recipient evaluation, intra-transplantation monitoring, and post-transplantation follow-up. However, lack of cases and post-transplantation monitoring in different periods may impact our findings. Post-transplantation monitoring in the same period can improve the comparativeness and accuracy of the study.

2 超声心动图对移植心脏形态、心功能及血流动力学变化的监测形态结构及心功能变化：通过对心脏移植患者超声心动图观察，发现移植后，特别是早期，右心明显扩大，右室内径与左室内径比值显著高于正常，对于该现象解释常认为是早期出现右心衰并发症以及可能与肺动脉压升高以及心肌水肿、缺血有关[16]。心脏移植后右心室增大有关影响因素有：①肺动脉高压，扩张型心肌病晚期心力衰竭导致肺淤血，长期的肺淤血使肺血管阻力增高，肺动脉压力增高。Murali等[17]报道随着肺血管阻力增高，心脏移植后右心衰竭的死亡率增高。②受者在心脏移植前心功能差，体内液体潴留多，心脏移植后大量的体液回流导致右心负荷加重，肺动脉压力增高。③心脏移植过程中心肌缺血、再灌注损伤、创伤性水肿均可使肺动脉压力增高。Valantine等[18]认为移植后近期心包积液是由于移植过程中主动脉及肺动脉广泛分离导致淋巴积液流入心包腔所致，此外还与移植切口的渗出有关，若积液量突然增加或持续性增加为急性排斥反应的重要特征。血流动力学变化：林雁娟等[19]研究认为心脏移植后血流动力学变化多发生在早期，主要为右心功能不全所致，及时监测血流动力学变化并分析其原因，可以正确指导临床治疗，对病情恢复及愈后起着重要作用。心脏二维结构与心肌功能的改变必然导致心脏血流动力学的改变。轻度排斥时二、三尖瓣血流峰值速度可无明显改变，但二尖瓣压力减半时间却显著缩短。从频谱上看，二尖瓣舒张早期血流速度急速上升，一方面反映了左房压力减半时间的缩短，另一方面也反映了舒张早期心室充盈加快。因此，二尖瓣血流动力学的变化可作为非常好的无创性排斥指标。超声心动图对供体心脏的评估：超声心动图是可以用来评价舒张功能的精确方法，不受左右室充盈情况的影响[20]。超声心动图的定量组织速度成像技术能对局部心肌运动进行定量分析，应变及应变率指标不受呼吸运动及心脏整体运动的影响[21-22]。 3 超声心动图监测心脏移植后排斥反应的应用价值潘翠珍等[23]应用Philips iE33超声显像仪，X3-1探头对60例心脏移植无排斥患者、10例心脏移植排斥患者、40例正常对照者进行三维超声心动图检查，结果见表1。 "

4 超声心动图对移植心脏冠状动脉病变的评估移植心脏冠状动脉纤维增殖性疾病已成为移植受体长期存活的主要危险因素之一，因而该病的诊断十分重要。血管内超声可非常敏感且特异的观察到整个血管壁全层的改变，可计算内膜横截面面积及内膜指数来反应血管壁的变化，进而预测心肌梗死、冠状动脉血管病变的发生，但血管内超声也是一种有创的方法且它不能观察所有血管的改变[26]。Akosah等[27]报道显示，超声心动图能准确辨认移植受体事件发生的风险，并有利于监测移植血管病变。Derumeaux等[28]认为正常超声心动图代表患者病情稳定，并可使其它有创检查推迟，超声心动图的预测值与血管造影剂血管内超声相当。 5 不同超声心动图对心脏移植监测的特点 5.1 常规超声心动图早期研究认为发生心脏移植时室壁增厚，心肌质量增加，左室舒张功能受损，等容舒张时间及二尖瓣压力减半时间缩短，左室收缩功能减低。但Dodd等[29]研究表明常规超声心动图对心脏移植急性排斥反应的敏感性低。发生急性排斥反应的心肌受损会引起心肌细胞的收缩功能障碍，左心室的收缩功能受损，但常规超声心动图检测的射血分数值是左心室的整体收缩，发生心脏移植急性排斥反应经常是局部的心肌受损，所以对左心室整体收缩功能不一定造成明显影响，而当受损心肌面积较大时即排斥反应已经累及大部分心肌时，常规超声能检测到左室整体收缩功能减低，但这个阶段将是各种临床治疗手段无法逆转的病理过程，其预后很差。因此常规超声心动图监测急性排斥反应的敏感性低，临床上需要一种在心脏移植急性排斥反应早期即能敏感的检测出相关指标的方法。 5.2 二维及三维超声心动图刘红云等[30]应用二维应变超声心动图检测移植心脏左室壁峰值收缩应变，探讨二维应变超声心动图评价移植心脏左室收缩功能的价值。将9例心脏移植受者共进行41次超声检查，23例正常人作为对照，记录心尖长轴观、心尖两腔观及心尖四腔观的高帧频二维图像，应用二维应变分析软件测量左室壁各节段的峰值收缩应变及心尖各切面观心肌总的峰值收缩应变、左室整体平均峰值收缩应变。结果认为，二维应变超声心动图可用于快速准确评价移植心脏左室收缩功能，在排异反应诊断方面具有潜在价值。但大多数学者认为，无论是单独或联合应用上述二维超声指标，都不能足够敏感的监测排斥的发生。实时三维超声心动图能够实时显示心脏的立体结构及功能状态，弥补了二维超声心动图扫查切面有限的缺点，可提供更为丰富的血流信息，更为详尽的进行血流的定性、定量分析，通过对心脏血流动力学的深入研究，可进一步了解心脏泵血功能情况，从而提供信息量更大、质量更高的图像，使测量更为准确。陈海燕等[31]入组健康志愿者60例及心脏移植患者31例，所有研究对象均行常规超声心动图检查，并应用单心动周期实时三维超声心动图评价右心室形态及收缩功能的相关参数，所有心脏移植患者超声检查3个月内曾行心内膜活检，根据1990年国际心肺移植协会排斥反应标准，将病例组分为排斥组和非排斥组。研究表明，右心室每搏输出量及右室射血分数在心脏移植对照组、非排斥组及排斥组间呈递减趋势，可以用于临床快速监测排斥反应。初步的临床研究已经显示出实时三维超声心动图独特的优势和良好的应用前景。Kapetanakis等[32]的一项较大样本量的研究证实了其临床应用的可行性和评价左室同步性的准确性。 5.3 多普勒超声心动图心脏移植急性排斥反应发生时心功能指标的变化早于超声可见的形态学变化，因为排斥反应早期，变态反应性炎症尚未导致超声可见的形态学改变。彩色多普勒能够提供心功能改变时血流的相关信息。Habib等[33]研究表明，以内膜心肌活检为标准，二尖瓣口舒张早期峰值速率与舒张晚期峰值速率比值增加>30%，发生急性排斥反应的敏感性为74%。Stengel等[34]研究发现，当多普勒舒张晚期峰值速率取最佳临界值<8.7 cm/s时，对心脏移植后严重急性排斥反应的检出敏感性和特异性分别为82%和53%。发生急性心脏排斥反应时左心室等容舒张时间及二尖瓣压力减半时间缩短。虽然左心室等容舒张时间与多种因素相互影响，许多研究结果显示这一参数对舒张功能诊断具有重要意义[35]。研究表明，多普勒超声技术所检测的左心室舒张功能参数可能用于监测移植心脏是否发生排斥，舒张功能减低可能是心脏移植急性排斥反应的惟一早期表现[36]。Valantine等[37]研究认为，心脏急性排斥反应时左心室等容舒张时间缩短是肺小动脉楔压增加，二尖瓣提前开放所致，其研究结果表明多普勒超声心动图测定的左心室等容舒张时间与病理分级有良好的相关性。 5.4 组织多普勒成像组织多普勒可直接从心肌组织的舒缩活动中提取多普勒频移信号，组织多普勒可以定位于局部心肌，定量测量局部心肌的收缩、舒张功能，所以组织多普勒超声心动图能早期发现舒张功能及收缩功能异常及早检测急性排斥反应[38-41]。在心脏移植后，应用组织多普勒成像技术定点、定量测量室壁运动及室壁运动速度变化，对早期检出及治疗排斥反应提供可能。 Dandel等[42]应用组织多普勒成像对移植心脏左室后壁基底段研究发现，舒张早期峰值速度在排斥组明显低于无排斥组，认为组织多普勒成像舒张功能参数可以对心脏移植后排斥反应进行无创监测。研究报道，发生收缩功能、舒张功能减低与急性排斥反应有关[43]。应用组织多普勒成像技术可以早期检测出舒张功能异常[44]，因最早病理改变发生在局部心肌时，整体血流动力学并未发生改变，直到病理改变发展到一定程度，才可以通过二尖瓣频谱观察舒张功能的改变，而组织多普勒成像可以定位于局部心肌，定量测量局部心肌的收缩、舒张功能，为早期发现舒张功能异常，及早防治排斥反应提供依据。因此，应用组织多普勒成像技术诊断急性排斥反应成为现在研究的热点。"
